If you have an NDIS plan, support coordination can help you explore providers and organise services around your goals. This may involve conversations about help at home, community participation, therapy or changes in living arrangements.

The supports available depend on your plan and circumstances. Coordination is different from directly delivering personal care, nursing or accommodation.

My Aged Care is the Australian Government’s starting point for exploring funded aged care. Its assessment process helps determine eligibility and the services that may suit your needs.

You can explore options yourself or with someone you trust. Ask how any proposed care supports the person’s independence, preferences and connections.

Before you choose a provider

Keep the person
in the conversation.

Prepare a short list of priorities. Ask providers to explain anything that is unclear, including fees, availability and what is outside their service.

  • What help is needed, and what can the person do independently?
  • What matters about language, culture, routines and relationships?
  • Who will deliver the care, and how is continuity managed?
  • What are the costs, agreement terms and cancellation conditions?
  • How can the person give feedback or raise a concern?

Talk with us about coordination, navigating services and communication between the people already involved. We discuss whether we can assist, how the work would be delivered and any fees before you decide.

This page is a guidance pathway. It does not advertise L3SC as a government aged care assessment service, a residential aged care facility or a provider of every type of direct care. Do not assume that an L3SC service can be paid for through an NDIS or aged care package; we confirm the arrangements first.

Are the NDIS and My Aged Care the same system?

No. They have different access processes and funding arrangements. If you are unsure which applies, speak to the relevant government service before making changes to existing supports.

Can I enquire for a parent, partner or participant?

Yes, with their consent or appropriate authority. Start with general contact details and the kind of help you are seeking. Please do not include sensitive care records in the website form.
