How this form works

The enquiry form prepares a message on your device. It does not submit the form to an L3SC database or send an email automatically. You review the message, open your email app and choose whether to send it. If you use the copy button, the message is copied to your device’s clipboard.

What to include

Your own name and contact details, the service you are asking about, your state or territory and a brief nonclinical introduction are enough. Do not include participant names, NDIS numbers, diagnoses, clinical reports or identity documents in this initial enquiry.

Consent and authority

If you are enquiring for someone else, confirm their consent or your authority to act. We will discuss any further information and the appropriate way to share it before a formal referral proceeds.

Email and website access

Once you send your message, it is handled by your email provider and the L3SC receiving mailbox. Email and website hosting involve their own technical processing. Avoid sensitive information in the initial message.
