Level 3 adds a specialist approach when complex circumstances create barriers to accessing supports. The focus is on understanding those barriers and building workable arrangements with the people you choose.

Is Level 3 the right fit?
Level 3 specialist support coordination must be included in the participant’s plan for this funded service. A diagnosis alone does not tell us the whole story. We discuss the barriers affecting support and whether our team can meet the person’s needs.

A Level 3 specialist support coordinator helps address complex barriers to accessing support. Work can include bringing services together, clarifying responsibilities and coordinating plans for hospital discharge, housing transitions or a breakdown in existing supports.
Can L3SC assist with hospital discharge coordination?
L3SC can discuss referrals involving hospital discharge planning and communication between disability, health and community services. We assess service fit, funding and capacity first. The hospital and treating clinicians remain responsible for clinical decisions and discharge readiness.
