Level 3 adds a specialist approach when complex circumstances create barriers to accessing supports. The focus is on understanding those barriers and building workable arrangements with the people you choose.

  • Clarify barriers, priorities and the roles of each service
  • Coordinate communication across disability, health and community supports
  • Support planning for hospital discharge or changes in living arrangements
  • Help organise relevant information for plan conversations
  • Strengthen continuity when services or circumstances change

Coordination does not replace clinical treatment, an emergency response, legal advice or the responsibilities of your direct care providers.

Before work begins, we discuss

  • The person’s goals, consent and communication preferences
  • Level 3 funding, management arrangements and service fit
  • Current providers, urgent gaps and agreed responsibilities
  • How relevant records will be shared appropriately

Is Level 3 the right fit?

Level 3 specialist support coordination must be included in the participant’s plan for this funded service. A diagnosis alone does not tell us the whole story. We discuss the barriers affecting support and whether our team can meet the person’s needs.
