Discuss the proposed transition, the services involved and the coordination barriers. Avoid including clinical reports or identifiable participant information in an initial general enquiry.
Discuss a hospital or health referralDiscuss how the participant’s goals, existing supports and recommendations may need to be brought together. Confirm consent before sharing participant information.
Start an allied health referralSupport a planned handover that clarifies participant preferences, outstanding actions, current provider contacts and agreed responsibilities.
Discuss a coordinator handoverDiscuss the participant’s preferences, communication needs and the authority or consent arrangements relevant to the enquiry.
Discuss the next stepDiscuss coordination needs associated with a move or changing supports. The participant’s choice of providers remains central.

Participant choice stays central
A referral does not commit a participant to L3SC or require them to use another L3SC service. Information sharing and ongoing contact with referrers are agreed with the participant or their authorised representative.
That is a separate service, with its own scope and enquiry pathway.
